Q: Do I need to fill out a health form?

A: Virginia Wesleyan Health Services must have a completed health form on file before a student is permitted to register.

Q: What if I can't find the health form that was sent in my deposit acknowledgement packet?

A: A blank health form may be downloaded from the Health Services Web site.

Q: Can I fax my health form?
A: No, we cannot accept faxed health forms. You can mail your health form in a business size envelope with two stamps. After sealing the envelope it should be pressed flat to avoid additional postage.

Q: Is it necessary to get a doctor's physical?

A: All new resident students  must have a medical exam and a completed exam form. Commuter students are not required to obtain a doctors physical but it is strongly recommended.

Q: Do I need to get all the shots listed on the immunization record?

A: No. Required immunizations are listed in bold print at the top of the immunization form. The other immunizations on the form are strongly recommended. Please have your doctor place his stamp at the bottom of the form.

Q: Where can I find my shot record?

A: Your physicians office or former high school should have a copy of your immunizations. A parent or guardian may have a copy of your childhood shots at home. Immunizations received at the health department can be obtained from their offices.
