A: No. Required immunizations are listed in bold print at the top of the immunization form. The other immunizations on the form are strongly recommended. Please have your doctor place his stamp at the bottom of the form.

Q: Where can I find my shot record?

A: Your physician’s office or former high school should have a copy of your immunizations. A parent or guardian may have a copy of your childhood shots at home. Immunizations received at the health department can be obtained from their offices.
